We are issuing this Notice of Privacy Rights to inform you of your privacy rights as an Animal ER client. We respect your personal information and are taking all reasonable precautions to meet the requirements of current privacy laws.

By providing Animal ER with your personal information, you consent to let us use your information for any of the following reasons:

• To process purchases from Animal ER on your credit card, debit card, or in the form of cash

• To follow-up with you on your pet’s medical condition

• To follow-up with your family Veterinarian on your pet’s current and future medical treatment

• To maintain your account

We will need to share your personal information with your family Veterinarian or other consulting veterinarian(s) outside the employ of Animal ER. Your information will be shared with third parties for reasons such as:

• To offer your pet continual medical treatment here and at another veterinary facility

• To provide accurate treatment options

Your personal information will only be kept by Animal ER for as long as required by law or until the information is no longer needed for the reason it was collected. Clients may request corrections or updates to their personal information maintained by Animal ER in writing.
